2012 MacBook Pro Completely Lost All Sound After Adobe Flash Install

PROBLEM SUMMARY: Installed flash on MacBook and then all sound immediately stopped working (safari videos and not even iTunes works, no startup sounds, nothing.)

MACBOOK SPECS: 2012 MacBook (Retina Display), OS X El Capitan Version 10.11.2, 2.5 GHz Intl Core i5, 8 GB 1600 MHz DDR3, Intel HD Graphics 4000 1536 MB

ADOBE FLASH SPECS: The latest version and all updates installed as of today Jan, 1, 2016

PROBLEM DETAILS/MACBOOK HISTORY: 1- I bought the MacBook used and completely formatted the hard drive and reinstalled the OS X etc. 2- I setup all the regular apps, logins etc. 3- I installed flash after about 3 weeks, the sound was working perfectly fine up until this point after which it stopped completely all output including to iTunes

FIXES ATTEMPTED SINCE THEN: 1- Uninstalled Flash (However couldn't find anything in Library folder left over from it to delete) 2- Updated to El Capitain OS X 3- Reinstalled the latest Adobe Flash and all updates 4- Did a PRAM reset 5- Restarted the MacBook

OTHER NOTES: The speakers are turned all the way up and nothing is muted, not even headphone jack output works. I've seen a few other ideas online but nothing so far has worked. Any thoughts, suggestions, ideas? Thanks!
