Q: "You shut down your computer because of a problem" every boot

For over a month now I have been getting the message,"You shut down your computer because of a problem" every time I boot my computer (I have not been shutting down because of a problem). I have a MacBook Pro (Retina, Mid 2012) running 10.10.5. I have also noticed my MacBook will not boot when connected to an external display via HDMI at startup. It will just stay on a black screen with a mouse cursor. If I boot first and then connect the display, I have no problems. I am not sure if this what is causing the error or if it related, but thought it worth mentioning.

I have tried the following:

Resetting the SMC

Clean install from 10.9.5 to 10.10.5 using time machine backup (I was going to update to 10.10.5 anyway, hence upgrade instead of reinstalling).

Booting in safe mode (The message did not appear for the next several boots, but shortly afterword continued to persist)

Disconnecting all peripherals

Resetting the NVRAM

Reinstalling the OS from the recovery partition - Before downloading Yosemite it gave me the error that my computer could not be verified and thus would not continue

Running apple diagnostics - This led to my Mac booting into internet recovery mode instead of running diagnostics

Any help or suggestions for this issue would be appreciated.

See if you MBP falls into this replacement program:

    https://www.apple.com/support/macbookpro-videoissues/

If so, bring it into an Apple store for testing.

    Ciao.

After entering my information it did not show my Mac as eligible for repairs. I do not believe my problem matches the symptoms listed as the blank screen only occurs when I boot while connected to HDMI. I also haven't noticed any unexpected restarts either.

See if you have a kernel panic report.  Use these directions top log and post it, if possible:

It appears that your MBP is experiencing some GPU problems and falls into this replacement program:

    https://www.apple.com/support/macbookpro-videoissues/

Make an appointment at an Apple store genius bar and request that they run a VST test.

    Ciao.

Thanks, I'll look into it. Are there any tests I can run at home to double check that the GPU is the problem before making the trek to the apple store to have them run their test?

I suppose you could run the Apple Hardware Test but they are not 100% reliable.  Let me point out that the 'trek. to the Apple store is in your best interest.  If the VST test that they will run indicates a failure, you will have a new logic board installed gratis instead of having to pay about $700+ for a replacement.  Note the expiration terms in the link.

    Ciao.

For those interested, I can now confirm that it is a graphics related problem. The problem is the use of external HDMI monitors. Preforming these steps on my machine as follows to get the error message and remove it.

    1) Boot - No error message

    2) Plug in HDMI

    3) Shut down

    4) Unplug HDMI

    5) Boot - This time there is the "You shut down your computer because of a problem" message. The message appears on every subsequent boot as well.

    6) Boot in safe mode

    7) Restart - The message does appear this boot or any subsequent boots

I also checked disk utility while receiving the message. After repairing disk permissions, it shows a problem with "private/var/db/displaypolicyd". Will be contacting apple shortly now that I can replicate the fault.

Also, thank you OGELTHORPE, I had not been aware of the faulty MacBooks until this thread.
