My first thought is to check to see if you have AssistiveTouch enabled on your iPhone. Take a look at the screenshots in the resource below. If this is not what you are seeing, perhaps you can reply with a screenshot from your iPhone. Another option is to see if there is an Apple Retail Store or Apple Authorized Service Provider nearby and let someone have a look at your iPhone with you.
