If this is true, then it means you are probably still running OS X 10.7 (Lion) that came with the machine, and you have not upgraded your OS X in the four years you have had your machine. That is the source of your issue.
You do have the most recent version of iTunes that your version of OS X can run. However, iOS 9 on your iPhone 6S requires at least iTunes 12.3.1 (current is 12.3.2). That version of iTunes requires at least OS X 10.8.5 (current is OS X 10.11.2).
You should always promptly update/upgrade Apple software.
Start by installing OS X 10.11 (El Capitan):
Upgrade to OS X El Capitan - Apple Support
Then update all your other Apple software from the "Updates" tab in the App Store.
Then you will be set!
