iPhone 6s Plus - Loud Hum with Headphones

My iPhone 6S Plus plays music fine through it's native speakers. When I plug in headphones, I only hear a loud hum. I've used the native apple ear buds and my Bose QuietComfort 15 headphones - same noise. Same experience using Spotify and Apple Radio. Any ideas? Thanks for your help.
