If everything is enabled:
On both your iPhone and Mac:
Sign out of iCloud
Sign out of iMessage
Sign out of FaceTime
Turn off Bluetooth
Turn off WiFi
Restart both devices
Turn on WiFi
Turn on Bluetooth
Sign into iCloud
Sign into iMessage
Sign into FaceTime
Then try to enable call forwarding and sms forwarding
