keychain login / icloud

I have recently upgraded to El Captian and switched to using iCloud keychain. I have an older iMac 27" early 2009.

Three Questions:

When I turned on iCloud Key Chain all the item I had in my 'login' key chain did not transition over to iCloud. Why would that all transition.

Types of items that did not transition are Secure Notes, and quite a number of passwords.

Also I am now having problems accessing all the items in the 'login' key chain. I know the password, that is not the issue. I have on rare occasions been able to access items in the 'login' key chain. There seem to be no rhyme or to why it will work or not work.

This would not be so bad but many of my 'login' key chain passwords where computer generated.

Finally, Where did the 'Key Chain Access' repair go to. It is no longer in any of the menus. Is this a side effect of using iCloud?

It doesn't automatically transfer all password items from the login keychain, though the documentation doesn't make this very clear. Secure notes are never transferred.

Keychain First Aid was eliminated as part of security update. It's no longer considered necessary.
