If you've forgotten your id : If you forgot your Apple ID - Apple Support
To reset its password : http://iforgot.apple.com e.g. by answering the account's security questions
If you also can't remember your answers then contact Support in your country to get them and the password reset : Contact Apple for help with Apple ID account security - Apple Support
If you still can't find your id, or if Support can't confirm your id and can't reset the password, then if you were the original purchaser of the phone (i.e. it wasn’t bought used) then you could try contacting iTunes Support online (http://www.apple.com/support/itunes/contact/) and see if they can remove the phone from the account - they are likely to want to see proof of ownership before they might help e.g. original receipt with the serial number on it
