I see that you have concerns about the performance of the receiver in your iPhone. Without some testing, it is not possible to say if it is software or a problem with the phone it self. I would suggest starting with the steps in this support article to find out what is going on:
If you can‘t hear a person on a call or voicemail or if the sound isn‘t clear on your iPhone
