A company phone was returned from a terminated employee. I cannot contact the employee to remove the find my iPhone. I do have the invoice with purchase date and the imei. I need the apple url in order to provide the info to Apple to have it unlocked. I have done this before, but cannot find the url to apply.

I cannot activate the phone because find my iPhone is still on it. This solution does not help.
If asked for a password, enter it. A password prompt means Activation Lock might be enabled in Find my iPhone.
I don't have the password
