Q: Find my iPhone on. I have invoice and IMEI number

A company phone was returned from a terminated employee.  I cannot contact the employee to remove the find my iPhone.  I do have the invoice with purchase date and the imei.  I need the apple url in order to provide the info to Apple to have it unlocked.  I have done this before, but cannot find the url to apply.
