Why am I getting an alert saying "enter passcode on other iPhone" when I don't have another Iphone?

My husband has an Android and recently changed my password on my IPhone, and set up some application on his Android, which I'm guessing, connected my phone to his, somehow. So when I went to change my password, I'm getting an alert saying: Please enter passcode for your other Iphone...." Then says something about how the other device (my husbands phone,) holds access and control, and I can't get back into my device without it. He supposedly doesn't remember the passcode and uninstalled the app, but I still can't get into my Account. Can anyone help me with this?

iPhone 6s Plus, iOS 9.1, Carrier: T-Mobile
