The missing index.xml dialog originates from one of Apple's iWork '09 applications that continues to run when a user double-clicks an incompatible document created by one of the current Pages v5, Numbers v3, or Keynote v6 applications.

The solution to this error condition is two-fold:

  1. Properly quit the iWork '09 application. It is not quit by clicking the red traffic light in a document window.
  2. Ensure step 1 is complete. Then, double-click the document to open it in one of the current applications that created it.

The iWork '09 applications create documents that contain an internal index.xml file. The current application counterparts do not produce documents that contain this file, and are not backwards compatible.

Less frequently, a user may rename an MS Office document to one with an iWork '09 document extension thinking incorrectly that this will convert the document.
