Unable to connect to wifi

Unable to connect to wifi, unless there is a user name and passwor, after 9.01 update. I have reset my everything including a factory restore, nothing has worked.

iPad (3rd gen) Wi-Fi + Cellular (VZ), iOS 9.2.1
