osx el-capitan, missing user account

I came to work today and attempted to log into my imac(27 inch. late 2013) and got the incorrect password shake. Tried a few more times, but it was the same thing every time. Wat. In the end i was 100% sure that my password was correct and started getting worried. I then booted into recovery mode to reset my password, but the account in question was not in the list. Only root and guest were there. More wat. Getting more and more worried i booted into single user mode to check if my files were there at least. And yes. The /Users/username folder was there, but the owner was set to 501. The same group was set, but my account was not the owner anymore. At this point i'm in panic/confuse mode and googling frantically on my windows box. What is going on? I couldn't really find anything.. Anyone with stronger google-fu than i have, please point me to what you could find.


I attempt to reset my password in singleuser mode. ( passwd username ).... No such username..


So i give up getting my old account back. It seems to be gone. I reboot and login as root. This seems to crash/hang a few times before i get in( i forced a few reboots). But i'm in. Still in system preferences > user and groups my account is nowhere to be found. So i create a new account with the same name. It asks me if i want to use the existing folder, i do, and it creates the account in a few minutes( time was spent chown'ing it seems). I reboot and log into the new-old account and the finder windows open as they would have in my old account. I start iterm and the empty window structure i know so well is there. This is clearly my old account. The keychain spam is real. something something does not exist. Reset? Yes. Yes. Yes.


I now don't trust my mac at all. I start backing up anything i don't think i already have backed up and might want in the future before i reset. Also all logs and crash-reports etc.


While thats going on its time attempt to figure out what happened. I have a few applications that log at hourly intervals running and they seem to have run up to about an hour before i attempted to login this morning. So it was running at that point. There are also a lot of crash reports from various osx processes. Some of these are empty files ( or more precisely files with only zeros. like 50K of just zeroes), but some contain stack traces.


The first one since i left the machine alone seems to be mdworker that crashed at 000700 and again 20 seconds later. Both 50ish K of zeroes.


Then a lof of mds_stores that continue crashing on 5 min intervals for about an hour. Mostly just zeroes but a few have traces like below:

They mostly seem to be related to calls about, or connected to, the filesystem. I also booted into single-user a few times and it could randomly hang on "still waiting for root device". I do get in like 2/3 times. So i'm guessing (again) that this is the harddrive thats failing or somesuch. fsck doesn't report any errors though.


In the end i'm able to login and 'use' it again. But there are a lot of random beachballs on applications from time to time. For instance when shutting down almost everything will beachball. mds_stores and mdworker are working hard. Even the activity monitor will beachball in the end. So i'm replacing it.


I would just really like to know what happened. If my user was actually bricked due to a hardware failure. So, anyone?

I have replaced the hard drive, and the RAM. I have tried restoring from an older Time Machine save point... I have gone into diagnostics mode, and done an extended hardware test. The test came back with no errors. I performed a file system repair (fsck -fy) and it claimed to modify some system files for me. But no matter what I do, I always get half way through boot, and it hangs.


So, in my mind, I have ruled out:

- Hard Drive

- RAM


What I am suspicious of:

- File system corruption

- Main Board failure

- (More specifically) Hard Drive Controller on Main Board


I am not sure what the FreeSegValidateLength call is in my crash report. I did a search for it, and found this post here as the only result in my request results. So, I am not sure what it is, what it does, and how to make it happy. It smells of memory and memory management. I am assuming it is a segment validator, possibly doing a pre-flight check on the hard drive or RAM, and it can't complete safely.


I have been working on ways to just log in. I have not seen a log in screen in days. Recovery Mode gets me the recovery interface... and Single User Mode gets me access to a terminal interface. I can traverse the file system in both, but I have not seen a successful boot to a user no matter what I try.


I am using El Capitan on a MacBookPro9,2 (2012 Mac Book Pro 12" / USB3)


This has been a sturdy MacBook... I really don't want to have to replace it. It was the last form factor I really enjoyed, having RJ45 and a built in SuperDrive. If anyone can lend support to this thread, please, I am really hopeful to get this machine back in production.
