Time to leave alerts sporadic

Is anyone else having issues with time to leave alerts in iOS 9? When me or my wife add entries to the family calendar some we both an alert and sometimes only one of us gets the alert. It seems very inconsistent.

iPad Mini, iOS 9.2.1, iPad Mini 2
