Can you post the output of the same command with count=5 ?

This is an JHFS+ volume. Can you check disk0s5 and disk0s6 to check what they are? You can try Testdisk, but there is not much chance you will get Windows back. El Capitan upgrade is unpredictable with a non-standard disk layout with three OSes.

At this point you can try Testdisk - http://www.cgsecurity.org/wiki/TestDisk_Download .

Please use /dev/rdisk0. If it asks for sudo, select it.

Testdisk is meant to be used to scan and discover partitions. It is not valid to change any partitions in Testdisk on a Mac. Do you see any MS Data entries in the Quick Search output?
