Syncing photos between Mac and iPhone creates duplicates

After taking a photo on the iPhone 6S+ and giving it time to sync with the Mac (OS X 10.11), I look back on the iPhone and it now has two copies - one of which I cannot edit or delete. I presume that the new secure copy is from the Mac.


So why doesn't the sync process from the Mac recognize that there is a copy on the iPhone and bypass copying the photo?

Why can't I delete the copy?


On a similar note, I chose to delete all photos on the iPhone, remove all duplicates from the Mac, and then sync up to the iPhone (with iTunes). None of the photos on the iPhone can be edited (without duplicating) or deleted (at all).
