Apple Hardware Test error 4SNS/1/40000000: VC0C-1.519

My Macbook Pro Core 2 Duo 2.53 13" MID2009 (4GB RAM, 320GB HD) looks like its working until I play any music.

When I play any type of music MP3 or MP4 its plays fine but after a while it start to hang. I have played it in iTunes thinking its maybe iTunes causing it. So I have downloaded VLC player and it does the same. So I thought maybe file is corrupted so I copied it on another MacbookPro and it plays normal without getting stuck/ hanged. So I have formatted my HDD and installed El Capitan but this was not a solution. I get the same think music get stuck/hanged.

I have run the Apple Hardware Test and got the error 4SNS/1/40000000: VC0C-1.519.

I am trying to find what this error means but can't find this anywhere on Apple not the Web.

Is there anyone here can tell me what is causing this? Could it be memory? hdd? or what?

Anyone has experience this or got a solution for me? What should i do?
