The Polar Beat app requires the app to be started first on your iPhone and your iPhone to remain nearby.

Regardless of any app's features, it is not presently possible to hang your watch on a treadmill or elliptical and monitor your heart rate continuously in real time. Any given app's features aside, Apple Watch does not presently offer an "always awake" setting for the screen. The maximum time period for which the screen can presently remain awake is 70 seconds. This is subject to the screen having first been tapped on and the Wake for 70 seconds option having been configured in the Watch app on the paired iPhone.
