Peculiar dialog PASSWORD_ALERT...

I am occasionally experiencing a situation where my system seems to go south, with one of the primary indicators being dialogs appearing that have labels like PASSWORD_ALERT_TITLE_WITH_USERNAME_OSX and dialog text of PASSWORD_ALERT_MESSAGE_WITH_USERNAME_OSX (see picture). As someone who trained as a developer, this is screaming that something is deeply wrong somewhere. When I google these labels, I get nothing.User uploaded file


When this happens, appears to also come along with a dialog ‘Keychain Not Found’ with text ‘Keychain “login” cannot be found to store xxxxx’. The xxxxx item can be different - not always same username (appears to be a username for one of the Internet services/web sites that I use).


If I reboot, these things go away - I do still have a login Keychain, it just gets lost temporarily.


One previous time this happened, I got a message that SecurityAgent quit unexpectedly, but did not see that this time.


Note that this system was done as a clean build for El Capitan (now on 10.11.3), running on a MacBook Pro Retina 13” Mid 2014 w/16 GB RAM. I am not happy with El Capitan - I like the features but find that the system becomes unstable. Typical symptom - happens much more often than this weird keychain/password alert issue - is that Mail seems to complain about Account Error. Then I may notice sluggish performance. Attempting to quit apps leads me to see the all the core Apple apps - Mail, Calendar, Messages, maybe Safari - are all refusing to quit. This seems to be worse after sleeping overnight. Have tried sleeping connected to Ethernet than WiFi to see if some issue with Power Nap. I realize this issue belongs in a separate thread but thought I would mention in case related to above.


In terms of any unusual systems software installed, the system is being managed with Casper Suite and has Symantec Endpoint running.


Would appreciate any solutions, clues or guesses on the peculiar dialogs (or the other issues I have mentioned). Unfortunately my experience has me hesitating to recommend El Cap to other people - would like to see my system become more stable again!

I just received this very same message today. My firewall is also reporting a virus on my network for the last month perhaps related, maybe not. I've run Sophos Anti-Virus (for Mac) scans which produced no alerts/results of bad software or apps loaded on my machine, but I also don't trust anti-virus software packages very much either for results. The scan flagged some other files which I know for certain are false positives.

I suggest each of you who has experienced this warning to start your own topic. Then download and run Etrecheck. Copy and paste the results into your reply. Etrecheck is a diagnostic tool that was developed by one of the most respected users here in the ASC and recommended by Apple Support to provide a snapshot of the system and help identify the more obvious culprits that can adversely affect a Mac's performance.

On reading your advice and having the same problem, I'm pasting the output of Etrecheck below. Note that I have already clicked the clean up link then clicked Remove to remove the orphaned file- I have no idea if this has any bearing on the cause of the PASSWORD_ALERT message.

BTW- I did not see anything strange in activity monitor that lead me to believe this was malicious. However, I still did not want to trust this.

I had Malwarebytes already, but installed its update, then ran it.

It found MPlayerX.app in my applications folder and its .plist file.

It suggested removing them.

Since it had an old copyright 2012, probably not using it, I just did remove it.

This happened to me for the first time this AM from running overnight. Same as other comments with additional observations. Desktop clear of all icons except Macintosh HD icon. When I tried a restart then a shutdown, it hung on Contacts. Contacts not only was not responding but it also would not do a force quit. Finally had to do a hard shutdown via the power button.


My suspicion is that whatever it is was attempting to raid my Contacts. Hopefully that didn't happen as I did not touch either choice.

Try running this program in your normal account, then copy and paste the output in a reply. The program was created by etresoft, a frequent contributor. Please use copy and paste as screen shots can be hard to read. On the screen with Options, please open Options and check the bottom 2 boxes before running. Click “Share Report” button in the toolbar, select “Copy Report” and then paste into a reply. This will show what is running on your computer. No personal information is shown.

I just got the same dialog.


This happened after updating some software, though I also had the system under load when updating, so it may very well be an internal issue with OSX. The software I was updating does some pretty low level in regards to virtualisation. I've run two sets of scans across the system to make sure it wasn't anything malicious. Probably worth asking if Apple have the text somewhere in their codebase, as it maybe a subsystem/keychain thing.

I don't recommend doing that, unless you've got some layers of advanced threat protection, including URL reputation filtering, payload sandboxing and end-point (process + memory) monitoring (not storage/hdd/ssd scanning).


Keep in mind, simply "viewing" an email can allow embedded malicious scripts to run. For this reason, I have hidden my "viewing window" in Apple Mail.

Viewing an email will download any html content which will notify the sender that you contacted their website. That will validate your email address to the spammer.

No other "malicious scripts" can be run when viewing an email.

I got the dialog just now for the first time on my Mac Air on High Sierra. It could be associated with the fact that I changed my Apple password recently. After changing that password, all my Mac devices seemingly went nuts, constantly requiring me to enter my new password and enter 6 digit codes sent to me. I ran and saved the results of EtreCheck. Then I shutdown all programs and requested the system to shutdown. It didn't seem to shutdown completely, though the screen was completely dark (no little circle rotating wheel thing). I finally held the power button down for 15 seconds, then released, then pressed it again. System came up, but said a couple of things indicating some kind of OS upgrade: "setting up analytics" (or something like that) and asking me if it's OK to send diagnostic reports to Apple. It's not unusual for my laptop not to shutdown. I haven't bothered to check into it, but so far it's always worked to forcibly power it off and on. Below are the results from EtreCheck:

I have now run into this on two Macs, the second replaced the first one when I couldn't stop it happening, and now a month later the second one is doing it too. Both are older machines with spinning HDs, and running High Sierra. That means they're slow, but it doesn't matter as all they do is curate a daisy chain of firewire disks used to store photographs, and run daily backups of one disk to another. In neither case did EtreCheck, Malwarebytes or Detect X show any malware, and Clam XAV says they're clean. EtreCheck did say I was having crashes of a process com.aladdin.hasplmd.plist in /Library/LaunchDaemons (this is some digital rights management process that could have got there from an installation of X-Plane with a USB key to confirm the app is registered, or more likely from X-Rite i1 software for a screen calibration device (though why that should need it isn't clear to me) and noted errors related to 'too many files open'. I see others have noticed that as well. I've deleted the files related to the HASP software and I'll see if makes a difference. The first mac to be affected by this had been doing that job without complaint for several years, and this began after it was 'upgraded' to 10.13. I see others have been getting this on older versions of the OS so that's probably not relevant. I'm thinking Garrett Davidson might be on the right track with his suggestion that some rogue app or process that crashes repeatedly is causing the error related to too may open files. Until we know for sure, simply restarting once a day might be a temporary fix.
