How much would I be paying a month for the iPhone 6S 64GB or an iPhone 6S Plus 64Gb if I traded in my current iPhone 5S 16GB in perfect condition?

It doesn't mention rates for 5S to 6S Plus with modifications of storage.
If you enter your data it will give you the cash value of your phone, which you can then apply to any purchase at an Apple store.
There has been an advertised program (one method appeared in email from Apple Store)
that implies you could qualify for the iphone for a low monthly rate and a second program
suggested you could get another iPhone every year at a monthly rate, once you'd paid for
one for a year; both (each in different email from Apple Store) suggested a tie-in with a
third party bank or finance provision. And to see the official Apple Store for details.
From one official announcement -- as I'd signed up to something, and get these in email:

Depending on where you are, what applies to each country, region, or market, these vary.
Perhaps contact the retail Apple Store or an authorized Apple reseller, or a cellular provider.
{After reading around, I see the content of one email offer is the same one as previously
supplied link to Apple, in earlier post. And I must have trashed all the other email offers.}
The other offer involves a value assigned to your old product and a percentage applied to new.
