How do I get APP activation code?

I updated my IPhone 5c to IOS9 because the App I wanted required it. I was able to install the App and monitored the phone during the installation. I didn't see any numbers, letters, etc. However, after I installed the App, it requested an "activation code." I tried entering my Apple password but that did not work. Again, I didn't see any type of "codes" when I was installing the App ... how/where do I get the activation code? From Apple? Or do I have to try and contact the App's sponsor?

You are apparently talking of the JMobile App going by your device specs...I would start by reading the app's full description & requirements in the App Store to make sure you didn't miss some kind of required step. Also try contacting the app developer (a link should be in the app's description in the App Store) if you can't find anymore info.

Just reading about the app now, it appears (going by one of the screenshots) that you need to set up an account...did you do so?
