iMac, El Capitan, seriously slow (Etrecheck report)

A few days ago, while in bootcamp playing games, my system started to have a serious lag problem. I shutdown the system and rebooted in OS X, only to be faced with an everlasting boot up time. I tried starting in safe boot and even then still had the same boot up times. Once in the system, everything was moving extremely slowly, producing the wait cursor after almost every action: clicking an option in the menubar, opening a new finder window/folder, openings apps, etc. It would take minutes for these tasks to complete so I decided to try to reinstall over top of the system. Which would produce an overnight wait because of the restarting and boot up, with the boot process still happening the next day. The most telling sign that something was wrong was when I was attempted a recovery boot, it would present a Mavericks recovery HD, even though I had El Capitan installed. It turned out to be reverting to Internet Recovery, bypassing my recovery HD. After a few days of trying this process, I finally yesterday had the idea to use the Mavericks recovery to install Mavericks on my external hard drive in a separate partition. Which worked flawlessly and the system had its normal quick response time to system actions. However, whenever rebooting, the Mavericks system faced the same boot up times as my El Capitan system, resulting in me putting that system in safe boot. From Mavericks, I was able to download the El Capitan update from the AppStore and create a bootable USB drive from it. Which still resulted in the slow boot up times for the drive. I came across the recommendation for Etrecheck on these forums and downloaded it. Once I safe booted in my El Capitan system, I used airdrop to transfer the file from my iPad to the system. With the file being only 4-6MB, it took about 6 minutes for the transfer to finish. Before running Etrecheck, I decided to use the terminal command for permissions repair and came back this morning to more reasonable response times, still lagging on apps like safari, but doable. Then I ran Etrecheck and here is the report generated. (This is a 27" iMac, 3TB and I ran across the Apple Spport page for recalls on those systems but mine was not sold after the period listed.) I realize that its throwing hard drive errors but I'm not sure what the problem is. I've ran multiple hardware tests and disk verifications and have not gotten any issues reported.

Adding to pinkstones - remember that your backup will likely contain errors ( because the drive is failing).

So its best to back it up, but then do a clean installation and manually move back stuff.

This is a problem with automatic backups/clones - they will overwrite the GOOD backup with the BAD (corrupt). Incremental backups overcome this, btu make it challenging to get "back to a good backup" while saving all the interim files.
