I understand that you are having a hard time hearing people form the receiver of your iPhone. Let's see if we can get that resolved.
What you will want to do is clear anything that is on the receiver that may be casing the audio to be blocked. I would also test with creating a voice memo and then playing it back to see if the issue is still present.
