Kernel Panic - Macbook Pro 15'' - Mid 2010

I'm german but writing this in english, because I hope to have more success like this. My Macbook startet having Kernel Panics every once in a while 3 years ago, but I could deal with it. Now it happens every 2 minutes after reboot. This is my recent log:

This is a clean Yosemite vanilla install, El Capitan (clean) used to be on my device two days ago, so i think I'm safe to say this is not a Software Problem. I just read that this is a usual Problem, and Apple used to replace macbooks with bad Graphics Chips. Right now it doesn't look like Apple still does replace those devices. (MacBook Pro (15-inch, Mid 2010) intermittent black screen or loss of video Quality Program has ended - Apple Support) - Like you can see the Programm has ended. Does anybody have any idea what i can do? Is it possible to replace my Graphics Card (NVIDIA GeForce GT 330M 512 MB) myself or do I have to get professional support. If anybody knows an easier way to get this fixed, then sending it to Apple and paying a lot, I would be sooo thankful.
