Is this someone else's Apple ID and passcode? If so, unless you can get that person to remove the phone from their account, the phone is now useless to you.

Then who is the "him" you haven't been in communication with?
If you can't remember your Apple ID, you don't have many options. Did your girlfriend buy this phone new from Apple or an authorized reseller? If so, she will need to take the phone and the receipt from the purchase to Apple. If she bought it used, you're out of luck. You have a brick.

You need more than the box. You need the receipt.

As you will need to communicate with Apple to resolve this, you will need to find a way. Apple does not do tech support by email. If you have an Apple Store that you can go to, that would probably be the simplest solution.
