Q: Facetime with iPhone 5 not working

I noticed that my Facetime v.3 stopped working recently.  I cannot call out nor receive call alerts.  I am running El Capitan 10.11.3 and have an iPhone 5.  I verified that the iPhone and Facetime and programmed to receive/send calls.  I did not see another post on this issue-please assist.

iPhone 5, OS X El Capitan (10.11.3), iPhone v. 9.2.1

    1. Please make sure you've followed the instructions in this support article. Note that both iCloud and FaceTime on the Mac must be signed in to the same Apple ID; those are two separate steps. Also note that the Mac and the phone must be connected to the same Wi-Fi network. Ethernet won't work. Trying to pair the phone to the Mac as a Bluetooth device won't work either.

    2. If you can make calls but can't receive them, open the Notifications pane in System Preferences and select Do Not Disturb. If any of the top three boxes on the right is checked, then all incoming calls will be blocked under the corresponding conditions. If you want to use "Do Not Disturb," but you also want to receive calls when it's active, check either or both of the two lowest boxes, according to your preference.

    3. No Continuity feature will work if either of the devices is connected to a VPN.

    4. If cellular calling still doesn't work, sign out of FaceTime and sign back in. You may have to do the same on the phone. The email address that you use as your Apple ID must be able to receive FaceTime calls.

    5. If you have more than two clients, and some of them are intermittently reporting that the phone is not set up to allow calls, try reducing the total number of clients to one or two.

    6. If you always get a "Call failed" notice when trying to make outgoing calls, then (according to a report) you may need to set a stronger password for your iCloud account. After doing that, sign out of iCloud and sign back in on all your devices.

Thank you for the detailed list to help get me up and running again! Since my post and before your reply, I went into Help on FaceTime and read that the program does not support iPhone 5. Is it possible this may be the issue?

the program does not support iPhone 5

I think that only applies to Wi-Fi calling, which is something else.
