fMy iPhone5s started to form these black spots after a chunk of screen fell off on the top right corner and these black spots started to appear and got bigger over time . After a week, while I was setting my alarm ,out of nowhere my screen turned black with color changing vertical lines. I still get notifications and other stuff . Do I need a new phone or can I get it fixed

You are kidding, right? The screen is damaged, deal with it. Either get it fixed or replace the phone. How could you even wonder what is wrong???
Yes, it's fixable. It's likely only the screen which is damaged (and the frame).
You can get the screen replaced. Apple won't replace it though as your frame is damaged. They will sell you a refurbished phone at a discounted price.
You can forgo post warranty support and take it to a reputable independent repairer who can fix it for you.
