To pay, just hold your iPhone near the contactless reader with your finger on Touch ID. Depending on your location (you must be located in a country where Apple Pay is available), Apple Pay can be used with supported debit cards, credit cards and/or store cards.
It is not possible to add either PayPal or your iTunes account to Apple Pay as one of your sources of payment.
If you already have a supported credit or debit card on file directly with iTunes (ie not via PayPal), you may be able to add that card to Apple Pay a little more quickly and conveniently than how you would add other cards.
To set up Apple Pay on your iPhone, follow the instructions here:

You will need a supported payment card from a participating issuer:

Your question (the title of this thread) referred to Apple Pay.
However, if you instead meant to ask how to set up your iPhone and iTunes / Apple ID account (using PayPal as your payment method) to make purchases in the iTunes Store, App Store, iCloud and for other Apple services, then follow the instructions here:

Video on link from that page is above and says Apple Pay. In the video it says/shows setting up by opening wallet app, choosing plus button, choose next
to add card already in your itunes account.You say Apple Pay does not presently support making payments using your iTunes account.
Regardless, is no add card option as in the video.
I want to have my iphone6 to be able to make purchases via my card already in my itunes account (Like it says in the video).

The information that I provided is correct. To recap:
- You cannot add your iTunes account to Apple Pay.
- Your iTunes account exists separately from any payment method that you may have set up for it.
- You may, for example, have a gift card credit balance on your iTunes account and also have PayPal or a credit card set up as a payment method to cover the additional cost of any purchases that are not covered by your credit balance.
- An iTunes account is not a supported payment method for Apple Pay.
- You cannot add PayPal to Apple Pay.
- PayPal is not a supported payment method.
- You can add debit cards, credit cards or store cards.
- This is subject to your card being supported by a participating card issuer for Apple Pay.
- Not all banks are participating issuers and not all cards are supported.
- If you happen to already have a supported card set up as your payment method for your iTunes account, then there is a shortcut process for adding that card to Apple Pay on your iPhone.
- This still only adds your payment card, not your (separate) iTunes account.

To be able to set up Apple Pay on your iPhone 6, you presently need to be located within either Australia, Canada, Mainland China, the UK or the US.
Your iPhone must also have that country as your selected region: Settings > General > Language & Region > Region - set as appropriate.
It must also be running a certain version of iOS, depending on the country (Australia: iOS 9 or later, Canada: iOS 9 or later, UK: iOS 8.3 or later, US: iOS 8.1 or later).
You will also need to follow the other instructions here:

- You cannot add your iTunes account to Apple Pay.
