Most of the time this happens when folks edit their primary apple id e-mail address without first signing out of iCloud on their phone - the phone stays signed in with the old e-mail adress. Here's the latest from apple If you can't activate your iPhone, iPad, or iPod touch after installing iOS 9.3 - Apple Support

If you can sign into iCloud.com AND see your phone under All Devices, you can remove it from account as a last resort.

I have this problem recurring over and over again. My wife's iPhone 6, iPad2 and MacBook are constantly asking for both Apple ID and iTunes Store ID. We finally ended up going to the local Apple store and making an appointment with on of the Genius Bar folk's to reset the IDs. Everything was back on-line for about 2 weeks and then it started again. If we enter the password(s) (it was written down and typed out in it's case sensitive form so I know for a fact it isn't being entered incorrectly) it gets rejected every time forcing a reset of the password AGAIN.
