It seems (correct me if I'm wrong) that I cannot get a non-standard keyboard layout in any country the same way I can choose from a range of layouts when configuring a new Mac(Book).
Still, is it possibly worth getting the refurbished model outside the Us and have someone change the keyboard to me to the US one, or it's not really possible or not worth the cost or hassle?

I think you need to contact Apple stores or services centers where you live directly yourself to have any worthwhile answers to your questions.

If your country has an Apple on-line store, there will be a dropdown option to select available keyboard types, during the ordering process.
See example below:

I think he knows you can do it for new ones, but he wants refurb.
It's hard to tell by the wording of his posting. It would seem that buying a used one would be a no brainer, a question not needed.
Replacing a keyboard would require replacing the entire top plate, something that could cost as much as a used or refurbished Mac.
I think he is trying to find out how difficult and expensive it would be to change the keyboard on a refurb bought in his country.
Is it worth buying refurbished Macs or Macbooks outside the US if I want the US keyboard?
