On the Internet it seems that older iPhone models would allow you to disable call waiting directly. I am not able to find anywhere on my 6Plus to do that. Is this something that I have to setup with the carrier?

Depending on your carrier and what network they run, yes, you may need to contact them about that.
With my iPhone 6s, using iOS 9.3 on AT&T in the USA, I have the ability to turn off/on Call Waiting in Settings>Phone.
In mine the menu does not have the choices that yours shows. I think it is a carrier dependent thing.
Yes it is, as Michael told you earlier. Not all carriers allow for call waiting, and not all that do allow for the user to make alterations. You need to contact the carrier to check with them on those options.
