Basically, the ailing Mac works like an external drive. As Niel mentions, when you get the new one and during the initial setup, you'll come to a point where the assistant asks if you want to migrate stuff from another computer. Plug the old Mac, in target disk mode, in with appropriate cables (best is Thunderbolt) and the new Mac will recognize that it has valid OS X content and let you choose what to move over (apps, files and user accounts).
