1. Only if you're concerned about Windows malware. Mac OS X doesn't handle that itself.
2. It can be trusted but is only useful in a limited number of situations.

It isn't bad, but it also isn't necessary. I would save your money and not buy or install any AV tool, cleaning tool, or memory restore tool. You don't need them. These are all activities handled quite well by OS X.
ClamXav is not in the same category as MacKeeper, which is just one of a number of popular scams.
Internet scams are extremely prevalent, and garbage Mac software that claims to protect it from "viruses" and malware is nothing new. Most of those products are malicious themselves. ClamXav is not malicious, nor is it particularly useful though. The problem is that third party software is not an effective tool to mitigate the various extant scams and other threats that will result in bringing misery upon yourself. Only you can do that.
