Q: Why do I have to buy the same book over and over for each device?
I bought a book for the iPad on the iTunes app, then when I wanted to read the same book on my iPhone, I had to purchase the same book again and then to read on my Mac at home I had to purchase the same book again. I thought once I purchased a book under my account that I could use it on all of my devices, but apparently not so. Is this how it is supposed to work? Having to purchase the same book for each device? If not, how does this work and how do I get a refund for the two book I've purchased extra?

When you buy an ibook it will be free to redownload on your other devices/computers, as long as the book remains in the store, it's the same version of the book, and you don't move countries (and you are using the same account that you bought it with).
How were you downloading it on your Mac ? Books that you can redownload will show in the Purchased link under Quick Links on the right-hand side of the iBookstore homepage in iBooks. They should also show in the ibookstore with Download instead of a price button.
You were charged for the same copy that you originally bought (there isn't ore than one version of the book in the store ?), and you are using the same account ? To contact iTunes Support : http://reportaproblem.apple.com
