Q: "Ask for Permission" no longer working on Kids ipad???

we had set up our child's ipad with Ask for Permission to download any apps, purchases, etc. Also to allow for certain content levels. All of a sudden it's no longer asking for permission, no error message, it's just allowing him to download. Anyone else have this issue?
