Apple ID Verification: Two Factor Authentication fail

iCloud on my Mac wants me to verify my changed Apple ID by entering the new password.

I enter the correct password and an authentication code is sent to my iPhone.

The dialogue box on the Mac instructs me to enter my apple ID again and the authentication code, but there are no fields provided to enter the requested information. The only options are Cancel or OK. cancel does the obvious and cancels the authentication process. OK just closes the dialog box and leaves me with nowhere to go to enter the code.

Running OSX 10.8.5 on Mac and recently upgraded to IOS 9.3.1 on the iPhone 6 with two-factor authentication enabled

iMac (27-inch Mid 2011), OS X Mountain Lion (10.8.5)

Me too! The two-factor verification process is a disaster in my opinion. I tried to use it on my Apple TV and at first no verification code was sent, and then when I got that to be sent, after entering the verification code, it kept asking for my Apple ID and password which generated another verification code! What a mess. It has to work better in order to be useful!

All have latest IOS and Sierra. I can use Macbook Air System preferences to get codes. These Macbook generated codes don't work ever. 2-Factor SMS Texted Codes do not show up on iPhones or iPad (WiFi only not 4G.) Sometimes 5S and 6Plus iPhones worked to set up that phone. I expect to get SMS on my phone and enter it on iPad. I get none therefore I have no successful 2-factor logins on iPad Air2.
