Apr 12, 2016 3:57 PM in response to weswornomby Linc Davis,An unexpected safe boot can happen when a wireless keyboard is turned off, or in power-saving mode, at startup. If you're using an older Apple wireless keyboard with removable batteries, please press the button on the side to wake it up before pressing the power button on the Mac. The current-model keyboard has an on-off switch in the back right corner.

Apr 13, 2016 6:39 AM in response to Linc Davisby weswornom,Thanks for your answer. I am using the Mac wired keyboard, but I do use a wireless mouse, not Apple. It is a Targus. Could a mouse cause it? The diagnosis code is ADP000 but the report says that it didn't find any issues. Thanks.

I don't know. The only way to find that out is to turn it off and use something else. See also:

That's normal.
