How can Apple gave someone a replacement phone under my apple care+ cover with my IMEI/serial numbers while I still have the actual phone? My warranty cover should be until Oct 2017 and I have never made a claim. However, when I tried to book for service with my IMEI number. Apple stated my phone has been replaced!!

Sorry - you posted in a user to user forum on iPhoto for IOS - no one here can answer any question about why APple did anything - there is a contact us link at the bottom of the page - use it and contact APple if you want answers from them or go into your local Apple Store

Did you ever post your IMEI/Serial number online? You have to keep these details secret or someone may use them claim ownership of your iPhone.

I am new to apple forum discussion, not sure where should I post it apart from here. Talked to apple they said my phone no longer has the apple care+ cover because it has been replaced. I will talk to them again and ask them how can they gave a replacement phone while I still have the actual phone. This is very werid to me.
