Q: How can Apple gave someone a replacement phone under my apple care+ cover with my IMEI/serial numbers???

Did you ever post your IMEI/Serial number online? You have to keep these details secret or someone may use them claim ownership of your iPhone.

Talked to apple they said my phone no longer has the apple care+ cover because it has been replaced. I will talk to them again and ask them how can they gave a replacement phone while I still have the actual phone. This is very werid to me.
