Must Change Passcode alert?

I received this alert on my iPhone twice in the past two weeks. Any idea why this happened? I have never seen it before and no one else I've talked to has seen it. Is it a virus or something? I am very confused and worried. See photo.

Do you know if this came from a text or from Safari or just popped up? Personally I would be reluctant to click on it although some have done that here with no issues. I would not respond to it and I would reset Safari as per below. Then wait and see if it happens again. I have been seeing more and more of these and at this stage I am not sure if it is a phishing scam or not.

Resetting Safari. Close Safari completely from the app switcher window by double clicking the Home button and swiping up the Safari preview pane until it disappears from the screen. You may have to look for the Safari pane by scrawling to the left. Then go to Settings > Safari > Clear History and Website Data. Then do a forced restart. Hold down the Home and Wake/Sleep buttons at the same time for about 15-20 seconds until the Apple logo appears.

Another Piece of the Puzzle Solved

My wife had this same problem, and I tried the same solution that's often suggested: clear the Safari cache and then do a hard reset. But I couldn't get to the Settings app because double-tapping the Home button didn't clear the prompt or let me reach the Home screen.

Solution:

Hold down the Home button until Siri appears behind the mischievous prompt. Then say "Siri, please open my settings." Andabingo!
