My phone is asking to be activated, I've used this phone for 2+ years.
Have I been hacked into?

Without warning, my phone displayed an activation screen asking for my Apple ID LOGIN WITH PASSWORD.. It would not let me out of that screen, tried turning off/on...still there.
Are you saying it will not let you enter your Apple ID and passcode?
NO. It is requesting that I enter Apple ID etc but I do not need to activate this phone, as I have been using it for 2+ years & have not requested Apple to "reactivate" this phone. It seems to be similar process as if I was setting up a new phone or previous owned phone.
It could happen if there was some sort of catastrophic software issue. I've had it happen once. Never figured out why. Just proceed with activating it. It's the only thing you can do. If you have made regular iCloud back ups, it should just be a questions of restoring from the most recent back up.
