    The big problem is your drive may be failing. Back up everything now. Use TimeMachine or a clone.(The clone will make life easier when you get the new drive.

    A couple of test you might put the drive

    !. Run Hardware Test

    Using Apple Hardware Test - Apple Support

    2. Run Disk Utility > First Aid from your Recovery Volume

    OS X: About OS X Recovery - Apple Support

    3. Run fsck from Terminal

    Thanks for response did Apple hardware test basic found no errors doing extensive test will see what it shows.

    I believe Etrecheck pulls from /Library/Logs/DiagnosticReports when reporting the number of hard drive errors. You can take a look there or in the Console utility for further information.


    Kernel disk I/O errors would indicate a data access problem. If you are lucky that problem is simply a permissions issue fixed by a disk repair. More likely more drastic measures will be required. Reformatting the hard drive and reinstalling the operating system was required when I last had I/O errors.


    Either way backup first ASAP! I do not see time machine configured, so unless you are using some other service your data could be at risk.

    Yes, it was also perfect exit code 0 and the volume that the hard drive was o in green

    Then, I'm stumped... go on using the drive but keep daily backups.

    The log extract shows that the startup drive is failing, or there is some other internal hardware fault. A negative hardware test means nothing. You can't fix the problem with Disk Utility.

    Back up all data on the drive immediately if you don't already have a current backup. There are ways to back up a computer that isn't fully functional—ask if you need guidance.

    Make a "Genius" appointment at an Apple Store, or go to another authorized service provider.

    If privacy is a concern, erase the data partition(s) with the option to write zeros* (do this only if you have at least two complete, independent backups, and you know how to restore to an empty drive from any of them.) Don’t erase the recovery partition, if present.

    Keep your confidential data secure during hardware repair.

    Apple recommends that you deauthorize a device in the iTunes Store before having it serviced.

    *An SSD doesn't need to be zeroed.

    Yes, your Hard Drive is failing.

    You haven't got Time Machine hooked up.

    Get yourself an External Hard Drive ($60-80) and back up EVERYTHING right now.

    Seriously. You're about to lose everything on there.

    The processes that are failing to load, indicate a failing drive.

     

    If it turns out that it IS something else, you're way ahead of the game.  Win-Win.

    The only relevant information that you've presented is the log extract. You're dealing with a hardware fault—probably, but not necessarily, in the internal drive. Nothing else matters.

    Your hard drive may be failing. Or the degradation of time, a crash, cosmic ray, etc has corrupted a file, partition map, or some such. Or plausibly something else. All we can tell you for sure is that I/O failures are usually hard drive related. No smoking gun for a precision fix, but  it does resemble a problem I once had.  An apple certified technician may be able to tell you more.

    Thanks for the help also for info this LCD was replaced 4/15/16 under warranty and when I reinstalled it after repairs that is when the beachball and also messages of you are not connected to the internet but refreshing establishes the connection ethernet is my connection
