How to permanently delete volume icon that keeps coming up in the middle of the screen

I have a volume icon that keeps coming up in the middle of the screen on my iPad. Sometimes it also starts music playing in the background. Doing research, I have found other people have the same thing happening. I can get rid of it by holding both on/off button and reset buttons at the same time, but that only gets it off of the screen until the next time it pops up. How do I get rid of the icon permanently????? I would think Apple should have a fix in the software to correct this problem.
