struggling with creating a survey/questionnaire on pages/numbers

Hello, I'm trying to create a survey for work as a exit-evaluation form for service users. It has to include several 'tick boxes' for each question, it must all fit on one page, and there are a lot of questions so will need columns / tables. I'm not sure how to format this. I'm not sure whether to use Pages or Numbers, or whether there's a template I can use. I have a MacBook Pro but am new to using apple software. Any advice / tips / pointers in the right direction would be very much appreciated!
