May 22, 2016 4:08 PM in response to gsspikeby Demo,I Have no idea what rebooting is to you but If you didn't try a forced restart, try that now.
To force restart your device, press and hold both the Sleep/Wake and Home buttons for at least ten seconds, until you see the Apple logo.
If that fails, try this. Reset all settings. You will not lose any data, but all of the device settings will have to be entered in the settings app again including WiFi network passwords. Settings>General>Reset>Reset all Settings.

May 22, 2016 4:30 PM in response to gsspikeby Demo,IF that didn't work, try resetting all settings. It's not as much work as a full restore? If that doesn't work, then take Rysz advice. Backup and restore. The restore the backup.

May 23, 2016 9:47 AM in response to Demoby gsspike,I found that my key pad wasn't putting in the numbers I typed. I verified it trying to text. Well anyway I tried to restore it 2 more times and it finally took. I reinstalled the update and now everything seems ok..

May 23, 2016 10:03 AM in response to Demoby gsspike,Don't worry about that I didn't expect you to be sitting around just waiting for my replies. It was the weirdest thing If I hit 2, 3 would appear on the screen. If I hit 5, 7 show on the screen and hitting 0, 8 and 9 show. Thanks again

Regarding the original issue, did you check Settings>General>Auto-Lock? I have mine set to 1 Minute. Yours sounds like maybe it got set to Never somehow.
On the matter of your phone not taking your passcode to Reset All Settings, have you tried your Restrictions Passcode? This can be different from the one you use to unlock your screen.
On the batter of having phone issues while on a call, Go to Settings>Cellular>Cellular Data Options>Enable LTE> [change to data only] then test to see if the issue persists. You may need to restart your iPhone after changing this one. Hold Sleep/Wake and Slide to Power Off. Wait about 8 seconds and then power it back on.
