I understand that the people you are calling can't hear you when you are making a call through your car. Let's see if we can figure out what is going on.
While you are on a call, see where the microphone is coming from. If you tap on the Audio button while on a call, you should have the options to send and receive the audio for the call. Take a look at the screenshot and article below for more details.

In the audio option I'm sending the audio to the car hands free device not to the phone. When I change the audio option to go through the phone I can hear people in the call and they can hear me, but the audio is only going through the phone not to the hands free system in the car.
