I clicked my safari app and this popped up? It crashes my safari. I reset my phone and deleted every password and app. Has anyone else seen this?

Yes, thousands of people have seen these bogus messages.
Double tap the Home button, then swipe upwards on the Safari preview.
Go to: Settings - Safari - Clear History.
Do not call the number.
