Have you tried an SMC Reset:

Follow instructions for the battery that type in your MBP.(removable/non-removable.
Also you can inspect the connector cable and port to make sure that the spring loaded pins move freely and do have debris in them.
Since you mentioned that it sometimes seems to connect the cable/connector may have an issue. (internal mechanical damage) you can bring it to an Apple store, they should be able to diagnose it for free.

If available, try another charger and/or try your charger on another Mac.
Go to System Preferences>Power and post the following from your MBP:
Make certain that the charger is connected:

Note that an error free AHT is not conclusive.
